This role requires a Social Work Qualification with a minimum of 2 years post qualified experience.
About the team
The team are responsible for the safe discharge of adult patients from Hospital, You will be responsible for the assessment of each patient ensuring that all their needs are met and that they can continue to live their lives as freely as possible. this position involves assessments from home as well as being present on the wards when necessary.
About you
The ideal Social Worker will be able to manage time effectively to prioritise their workload as well as have good interpersonal skills. Experience working with adults, the ability to work in a fast-paced environment and the ability to be able to engage with service users from diverse backgrounds is key to the success of the role. A degree within Social Work (Degree/DipSW/CQSW) with a minimum of 2 years years experience is essential in order to be considered for this role.
