This team undertakes high quality Care Act compliant reviews and re-assessments. A key aspect of this position is to use appropriate risk management and an MDT approach when dealing with each individual service user. Advocating for service user independence when appropriate will be imperative – especially when presenting care needs at commissioning panels.
About you
The successful candidate will be a professional well versed in completing personalised care plans and re-assessing pre-existing plans in a timely manner.
