We are currently looking for an experienced social worker to join a busy hospital discharge team.

About the team
This team works closely with the multidisciplinary team to complete care act assessments, mental capacity assessments, and strengths-based care planning for adults preparing to leave hospital. You will coordinate discharge arrangements, D2A pathways and make sure that patients receive timely discharges. The team’s ability to manage a fast paced, everchanging caseload is key to the success the team has had.
About you
A successful candidate will have extensive experience working autonomously and effectively within a multidisciplinary environment. Experience working in hospital discharge or short-term intervention roles lend well to the success of this position. A degree within social work (Degree/DipSW/CQSW) with a minimum of two years’ post qualified experience is essential for this role (a completed ASYE is counted as one complete year). A valid UK driving licence and car is not essential but desirable for the role.
