We are looking for a Hospital Social Worker at St Richards Hospital
This role required a Social Worker Qualification with a minimum of 2 years post qualified experience.
This job requires a mixture of hybrid working and in hospital requiring of 2 - 3 days present in office a week. You will be required to attend the hospital to support with duty and visit the wards to undertake assessments and attend family meetings. We also work as part of the multi-disciplinary team covering A&E and the emergency floor.
About the team
The Hospital Social Work Team assesses people currently in hospital and after they have been discharged to identify what care and support they need. The team helps people maintain their independence and works closely with healthcare professionals, community services and voluntary organisations. It is a fast-paced role with opportunities to learn, gain experience and develop as a practitioner.
About you
Social Workers must hold a Social Work qualification (Degree/DipSW/CQSW) and have a minimum of three years’ post-qualifying experience. A valid UK driving licence and access to a vehicle are also required.

Identify what care and support they need long term.
Help people remain as independent as possible.
Work in a busy, fast-paced environment where priorities can change.
Work closely with doctors, nurses, therapists, community services and charities.
Workers who can communicate well, prioritise tasks and make good decisions.
Provide a supportive environment where staff can learn and develop their social-work skills.
Assess patients in hospital and after they have gone home.

About this role
This team works with individuals aged 18+ in hospital settings ready to be discharged. A key responsibility is to ensure safe, person-centred discharge from hospital by assessing needs and arranging the right support to promote independence and wellbeing. The team will be required to carry out face-face assessments, Care Act 2014 and MCA assessments.
About you
It is essential to have a degree within Social Work (Degree/DipSW/CQSW) with a minimum of two years' experience within adult's Social Work post qualified. Experience working in a hospital, CHC or short term assessment team lends well to the requirements of the role. A valid UK driving license and regular access to a vehicle is essential in order to be considered for this role.
