Hospital Discharge Social Worker

We are recruiting for a Qualified Social Worker to join an Hospital Discharge Team.

About the team
This team supports individuals and their families or carers to adjust to illness, access support and plan for any post-discharge resources they might need.

This organisation is committed to supporting employees to work with agility, where appropriate for the post and service requirements.
About you
The ideal candidate will have good post-qualifying experience in Adult Social Work.

Experience within Hospital or Intermediate Care lends well to this position.

A degree in Social Work (Degree/DipSW/CQSW).

Have a Practice Educator qualification.

You will also need to be Social Work England Registered.
