There are two vacancies within this Authority.

One in the Homefirst and Urgent Response Team.

The second is in the Discharge Hub. Both are busy and fast paced teams and would require a dynamic worker with experience supporting hospital discharge discharge and admission avoidance.Working arrangements: The team are based in the office 2 days a week, but expected to complete urgent visits at short notice.
