Leaving Care Support Worker - Children’s Services - Bolton Are you a passionate, dedicated Leaving Care Support Worker looking for your next role? 4Recruitment Services are recruiting a Leaving Care Support Worker to join a Leaving Care Team based in Bolton. Umbrella Rate: £21.21 per hour Duration: 3 months initiallyHours: Monday to Friday, 9am – 5pm, 37 hrs/week – Hybrid Leaving Care Support Worker role: Full JD Available
Direct work with Looked After Children, Care Leavers and Homeless Young People aged 16 – 25, developing their pathway into independenceTake responsibility on a day-to-day basis for your individual caseloadHome/ Community visitsSupport to improve life chances and outcomesOffer assessment and support to young homeless peopleSupport to access accommodation, employment, education Support with independent living skills such as budgeting, cooking, cleaningAssess and implement plans to address behaviourAssess their individual needs with a course of actionPrepare, produce, implement and evaluate plansPrepare for and participate in decision making forumsWork with partner agencies and colleagues to provide appropriate servicesMaintain appropriate records of services provided to service usersAttend reviews, case conferences, pathway planning and meetings where appropriate
Essential Requirements
UK Driving Licence, access to a vehicle and business insuranceRecent extensive experience working with young people leaving careAbility to undertake assessments and review programmes, including the provision of pathway plansKnowledge of issues facing young people leaving care or becoming homelessAbility to engage with resistant Young PeopleAbility to visit clients in the Bolton area Excellent written and verbal communication skillsYou Must Have Right to Live & Work in the UKEnhanced DBS registered to the update service
