We are dedicated to providing access to support that protects health and motivates individuals toward their own recovery journey. Role Overview - Essex RECONNECT Coordinator:In this fantastic role, you will be responsible for delivering a comprehensive, all-encompassing, and tailor-made RECONNECT service, which encompasses offering advice and guidance to clients of the Forward Trust who are transitioning from HMP Chelmsford to Essex upon their release.

Additionally, as the RECONNECT coordinator, you will assume responsibility for the oversight of the peer supporter initiative at HMP Chelmsford, with the added opportunity to receive training to assist participants in obtaining their OCN qualification.As the lead for the peer supporter project, you will play a pivotal role in liaising with prison departments to facilitate the recruitment and execution of their interventions. Role Responsibilities – Essex RECONNECT Coordinator:
Collaborating with the RECONNECT Team Leader to implement a comprehensive and robust strategy in accordance with the RECONNECT contract.Cultivating and sustaining strong partnerships with external agencies and key stakeholders.Recruiting and nurturing volunteers for the RECONNECT program.Establishing a clear referral pathway that includes Offender Management in Custody, Family/Self, and Prison Healthcare.Ensuring the safe delivery of a holistic service to our clients, aligning with individual and team goals set by the Team Leader.Collaborating closely with colleagues to exchange best practices and actively participating in risk assessment processes to provide a high-quality service securely.Guaranteeing that service users are effectively connected with local resources in preparation for their reintegration into the community.Assisting in the recruitment, training, and facilitation of peer supporter interventions at HMP Chelmsford.Supporting the recruitment, training, and retention of RECONNECT staff and trainees.Assisting the Team Leader in collecting and organising year-to-date data, which is to be submitted on a monthly basis through a data collection template.
 Person Requirements:
Demonstrated expertise in conducting thorough assessments and crafting and executing SMART care plans.Proficiency in providing structured interventions to individuals receiving services.Application of motivational interviewing techniques in both one-on-one and group contexts.Utilization of proficient IT skills to aid in project management.Adaptability to accommodate the evolving requirements of the service.Capability to represent The Forward Trust in a constructive and polished manner.Proficiency in effective communication with a diverse array of stakeholders and in various communication formats.A Diploma in Health & Social Care or equivalent.Relevant qualification to the field of mental health or substance misuse.
