Hospital Discharge Social Worker

Tower Hamlets Borough Council are looking for an Adult Social Worker to join their Hospital Discharge Team.

The Social Worker will be required to work on site and at home with the expectation to carry out face to face visits.

Your responsibilities as a Social Worker:

Implement care plans and care packages
Carry out MCA and Care Act 2014 assessments
Undertake Section 42 enquires
Working towards discharge clients under the Discharge to Assess criteria (D2A)
Working towards KPI's

Requirements of a Social Worker:

Current registration with Social Work England
2 years post qualifying UK experience
Qualified Social Worker BA/DipSW/CQSW or equivalent


Location: South-East England, Surrey
Umbrella pay rate: £36.00 p/h
