Occupational Therapist - Permanent - Batley

We are looking to recruit for a band 6 Ocupational therapist for a Community Partnership based in Bradford.

Our Urgent Community Response team, part of our Integrated Community Services, is comprised of Nurses, Occupational Therapists, Physiotherapists, Advanced Clinical Practitioners, Assistant Practitioners and Healthcare Assistants.

An opportunity has arisen for a motivated Occupational Therapist to join our team.

The service covers Community settings and offers a 7 day service, from 8am until 8pm, providing assessment, treatment and evaluation of clients’ needs, in a short term intervention program.

The team’s aim is to prevent unnecessary admission to hospital and provide Urgent Community Response.

You will have excellent communication skills to build complex relationships and network with key stakeholders to enable integration.

Using initiative is essential, as is skill sharing with colleagues within the team, and the wider organisation.

It is essential that all the team members work closely together, to truly integrate an MDT and provide a comprehensive service, utilising the Calderdale framework.

There are opportunities for therapy skills development and further education opportunities.

Main duties of the job

- To contribute to a holistic assessment for adults with complex physical and medical needs, including those with falls risk, neurological, musculoskeletal and frail conditions.

The services’ purpose is to work with key stakeholders and community services to avoid hospital admissions and assist in the timely discharge of those receiving care, providing support discharge planning and oversight to prevent avoidable re-admissions.

- To take part in the management of clients over the age of 18 requiring a support discharge from an acute setting, potentially with a complex health and social needs in the Discharge to Assess facility and managing complex discharge planning across Kirklees.

- To provide assessment, treatment and evaluation of clients’ needs.

The ultimate aim being to avoid unnecessary admission to hospital from the bed base, community and in addition to ensure the swift appropriate discharge of clients who need to be at home.

- Work with an MDT approach with community partners to avoid inappropriate admission to long term care.

- Provide advanced nursing Occupational Therapy assessment to patients with complex physical and psychological symptoms across a variety of conditions/ diagnoses in conjunction with colleagues.

Qualifications

Essential

- Registered: HCPC
- Awareness of Intermediate and Urgent care agenda
- Valid UK Drivers Licsense 

Desirable

- Experience of working in community services
- Clinical Educator Course
- Knowledge of inter-disciplinary working
- Clinical Supervision qualification

Experience

Essential

- Good organizational skills
- Ability to identify potential for rehabilitation through assessment
- Ability to prioritize and manage own caseload
- Be able to use research, reasoning and problem solving skills to determine treatment
- Ability to use range of IT systems
- Able to promote Service with support from

Desirable

- Team leadership experience
- Experience of involvement in Clinical Audit
