Adults Social Worker

We are looking for a Social Worker to join a community Learning Disability team in South London.

This role requires a Social Work Qualification with a minimum of 3 years post qualified experience.

About the team

Work as part of a multidisciplinary team supporting adults with learning disabilities, contributing to reviews and coordinating person-centred support planning. Complete Care Act 2014 assessments, reviews and support plans, ensuring eligible needs are met in the least restrictive way and independence is promoted. Undertake mental capacity assessments and best interests decision-making under the Mental Capacity Act 2005, including complex decisions relating to care, residence and finances. Extensive joint working with health, providers and families, coordinating packages of support across supported living, residential and community settings. Manage safeguarding concerns under Section 42 of the Care Act 2014, applying Making Safeguarding Personal throughout. A role suited to a social worker confident in the Care Act, the Mental Capacity Act and DoLS, multi-agency coordination, and person-centred, strengths-based practice.

About you

A degree within Social Work (Degree/DipSW/CQSW) with a minimum of three years experience is essential in order to be considered for this role. Experience within a learning disability or community adults setting is desirable.
