Key Responsibilities:

Providing person-centred support tailored to individual needs Assisting with personal care where required Supporting service users with daily living skills and independence Administering and recording medication (where trained) Promoting choice, dignity, and inclusion Supporting with care planning and documentation Liaising with families and external professionals
