We are looking for an organised and proactive Care Coordinator to join our client’s friendly and dedicated team based at their Dereham office. About the Role
As a Care Coordinator, you will be responsible for planning and coordinating care packages, ensuring that the individuals we support receive consistent, safe, and effective care. You will work closely with our nursing team, support workers, service users, and external professionals to maintain excellent standards across all services.

Key Responsibilities
Coordinate and manage care packages to meet individual needsCreate and maintain staff rotas, ensuring continuity of careAllocate support workers to assignments based on skills and availabilityAct as a point of contact for service users, families, and staffSupport the onboarding of new care packagesLiaise with healthcare professionals, commissioners, and internal teamsMonitor service delivery and respond to any issues or changes in care needsMaintain accurate records and ensure compliance with company policies and regulatory standardsSupport recruitment, onboarding, and ongoing supervision of support staffWhat They are Looking For
Previous experience in a care coordination, scheduling, or similar role within health or social careStrong organisational and time management skillsExcellent communication and interpersonal abilitiesAbility to work under pressure and manage multiple prioritiesA good understanding of person-centred careIT literacy and confidence using scheduling systemsA proactive and solution-focused approachDesirable Skills & Experience
