You will encourage patients & carers to engage with self-management by offering the support that will enable them to feel confident in managing the condition.As a Lymphoedema Specialist Practitioner you will work autonomously, managing your own caseload of patients, often with complex physical and emotional needs.You will:
Assess, diagnose, and agree management plansAssist in the delivery of management plansSupport the team in developing their skillsOffer the opportunity to treat and assess patients in their own homes as and when required
Person specification:You will be an NMC registered Nurse/HCPC registered Physiotherapist or Occupational Therapist with:
Specialist Lymphoedema experience ORExperience in related area (such as palliative care, oncology/other long-term conditions)Proven experience in managing patients with complex issuesHave access to your own mode of transport
