Social Worker

We are looking for a Social Worker to join the Reablement Team.

This role requires a Social Work Qualification with a minimum of 2 years equivalent post qualified experience.

About the team:

The reablement team works with individuals who has suffered an injury/illness to help regain their independence. You will work collaboratively within a multi – agency setting, the aim is to deliver short term goal – orientated interventions that promote recovery and reduce reliance on long term care services. This role is a short – term case holding opportunity.

About you:

To be qualified for this role you must hold a Social Work degree and be registered with Social Work England. You should have experience working within an adult short – term case holding team, with a solid understanding of relevant legislation.
