Responsibilities:

Ensure accurate and timely recording of data and updating of care plans using SystmOne To be responsible for the assessment, planning, implementation, and evaluation of evidence based individualised programmes of care. The post holder will also co-ordinate, manage and organise the nursing caseload. To recognise changes in patients’ conditions which require the intervention of others and refer on as appropriate.

Qualifications and Requirements:

Fully enhanced valid DBS Must have community experience

We offer a £250 bonus for starting a contract with SCS.
