Responsibilities:

Act as a link between primary care, secondary care, mental health services, social care, and voluntary organisations. Monitor patient progress and arrange necessary follow-ups or reviews. Develop tailored care plans in collaboration with patients, their families, and healthcare professionals to meet individual health and social care needs. Identify patients at risk of deteriorating health using risk stratification tools and ensure they receive targeted, proactive care.

Qualifications and Requirements:

Hold a degree in mental health nursing. Fully enhanced valid DBS Community experience is essential

£27ph LTD | £24.29 PAYE Inclusive | £21.68 Exclusive

We offer a £250 bonus for starting a contract with SCS.
