Qualified Social Worker
Social worker required for locality safeguarding team.To provide Social Work and Care Management services to service users within a defined geographical area and to be involved in the development of services and staff.
To co-ordinate the assessment of needs of designated service users.
To work with service users and their carers in accordance with agreed multi-agency assessment processes and hospital discharge procedures.
To undertake Safeguarding Adults Enquiries in line with the Care Act 2014 and Derby and Derbyshire Safeguarding Adults Policy and Procedures.To promote Self Directed Support to meet Care Act eligible needs.
To ensure that care plans are produced and to arrange or commission the provision of services appropriate to meet the assessed needs.
To have knowledge of a wide range of services and other resources, including statutory, voluntary and private provision.
To assist the Team Manager and Social Work Senior Practitioners in the development of policy and practice standards and budget monitoring.
It will be necessary to work with information technology and associated systems and existing technology agreements and consultative procedures will be applied in accordance with City Council policiesTo work with local people, their carers and their advocates, where appropriate, in the assessment of individual needs and the provision of appropriate social work and care management services/resources.To act as the primary point of contact for care arrangements in respect of designated service users, working in accordance with agreed joint policies and procedures. To identify the aims and objectives of proposed interventionsTo identify unmet needs and to record on departmental information system. To use a personalised approach when assessing and planning care and encourage uptake of Self Direct Support through a Direct Payment. To produce care plans and ensure their effective implementation. To be involved in the direct management of certain care packages, providing services within an agreed budget for the delivery of care. To ensure that the quality of service provided meets agreed quality standards. To maintain effective contracts with all providers of care and other people responsible for relevant community resources and to undertake joint work with other Social Workers/other professionals in the completion of tasks, initiating, where necessary, multi-disciplinary meetings for the co-ordination of complex care arrangements. To undertake a financial assessment role where required.To undertake Safeguarding activities as required in line with the Care Act 2014To act as a Duty Officer as required. To assist in the induction of newly qualified/appointed Social Workers/Care Managers.To provide consultation to other social work staff on a particular specialism.To participate in local planning groups/local practitioner groupsTo supervise Social Work/Care Manager students and trainees.To record all work undertaken and prepare reports/assessments.To accept responsibility, with the Team Manager, for reviewing work on a caseload at regular intervals.
This would include closure and transfer of cases.To seek to ensure income maximisation for people.Understand and comply with the City Council’s environmental policies.
