About the Role: Service Care Solutions is recruiting a Band 5 Community Nurse to join the team in Kidderminster NHS on an agency contract. Key Responsibilities:
Deliver high-quality nursing care to patients in a community setting.
Administer medication and treatments in line with EPMA protocols and training.
Provide assessments, care planning, and regular evaluations to meet patients' individual needs.
Work collaboratively with multidisciplinary teams, including GPs, physiotherapists, and social workers, to ensure seamless patient care.
Support patients and their families with education, advice, and emotional support regarding their care plans.
Manage a caseload of patients, prioritizing and organizing work effectively.
Maintain accurate and timely records of all patient care, adhering to professional and legal standards.
Participate in team meetings and contribute to service development initiatives.
Requirements:
Registered Nurse (Band 5) with a valid NMC PIN.
Previous experience in community nursing is highly desirable.
EPMA training is essential.
Strong communication and interpersonal skills.
Ability to work independently as well as part of a team.
Flexibility to work both day and night shifts, as required.
Working Hours:
3–4 shifts per week, including a mix of days and nights.
