We are looking for an experienced and compassionate Care Coordinator to join our team on a 12-month fixed-term maternity cover contract, starting in September.
This is an exciting opportunity for someone who is passionate about building strong partnerships across health and social care to ensure people can return home safely and receive the support they need.
The primary focus of this role is to work closely with Great Western Hospital (GWH) and community healthcare professionals, coordinating referrals and supporting smooth, timely transitions from hospital to home. You will play a key role in developing and maintaining professional relationships with discharge teams, community services, and local healthcare partners, ensuring individuals receive responsive, person-centred care when they need it most.
You will work collaboratively with a wide range of professionals including Hospital Discharge Teams, Social Workers, Occupational Therapists, GPs, District Nurses, Community Healthcare Teams, and other partner agencies to coordinate care packages, respond to referrals, and promote positive outcomes for customers living independently at home.
Alongside this, you will support the day-to-day coordination of our domiciliary care service, helping to ensure high standards of care delivery and continuity for both customers and care staff.
Key Responsibilities
Hospital & Community Coordination
Build and maintain excellent working relationships with Great Western Hospital, community healthcare teams, and local partner organisations.
Coordinate referrals from hospitals, healthcare professionals, and community services to ensure safe and timely care at home.
Liaise daily with Social Workers, Occupational Therapists, GPs, Hospital Discharge Teams, District Nurses, and other healthcare professionals.
Support hospital discharges by arranging care packages quickly and efficiently.
Ensure all referrals are managed professionally, with effective communication throughout the customer's journey.
Promote collaborative working across health and social care services to achieve the best possible outcomes.
Care Coordination
In addition to the above, you will also:
Develop and maintain efficient staff rotas that promote continuity of care and minimise travel time.
Match carers with customers based on skills, experience, and individual needs.
Manage last-minute changes, emergency cover, and staff absences.
Act as a key point of contact for customers, families, and care staff.
Respond promptly to telephone calls, emails, and messages.
Record and escalate concerns, complaints, compliments, and safeguarding issues appropriately.
Work closely with the Registered Manager and wider care team to maintain high standards of service quality.

What you’ll do at work:
To provide support with aspects of daily living such as person centred personal, practical social and emotional support as detailed in the care plan. Promoting independence, dignity and encouraging people to do as much as they can to maintain their abilities. Provide emotional support and social contact to reduce isolation and maintain quality of life.
Main Duties:
To read the care plan, risk assessments and protocols for the individual person before providing support
To log in and out of the person’s homes using the Carefree go app and ensure that all EMAR charts are signed if relevant and care notes are completed
To always follow the policies and procedures of Portfolio Homecare
To portray a professional image and represent the company in line with the Company’s high standards at all times
To provide personal care: assisting with washing, bathing or showering, shaving, oral hygiene, dressing and toileting including continence support
Helping with moving and handling, using equipment safely, observing changes in health, and reporting concerns immediately to managers and professionals
Preparing meals and drinks, support with eating and drinking, monitoring intake where required
Medication: Prompting, assisting or administering medicines in line with care plan and training. Following company policy and recording accurately on the EMAR chart on the app
Light domestic work such as laundry, shopping and helping to maintain a clean and safe environment
Completing visit records on the app and communicating effectively with colleagues, families and other professionals
To report any concerns immediately
To complete mandatory training and attend training sessions to ensure that you have the knowledge and skills to support the people in their own homes
To provide social support such as talking and listening to people who use our services, helping them maintain contact with family and friends
Enable people who use our services to maintain contacts in potentially isolating situations
To report all changes in the health and social circumstances of people who use our services to a manager as soon as practical
To attend supervisions, appraisals and team meetings when required
To read policies and procedures and to inform the trainer if you do not understand
To work as part of a team with colleagues and work closely with other agencies and professionals involved in the care of people who use our services, for example District Nurses
To plan your annual leave and to request a minimum of a month in advance for a week or more of leave. Maximum two week at a time. Minimum of a weeks’ notice must be given for a day off
Please remember annual leave is granted on a first come basis so if there are other colleagues off you will be given alternative dates
Take all annual leave within the leave year April 1st to March 31st as it will not be carried over
To ensure that your care is road worthy has an in-date MOT, car insurance is business insurance and is in date
Ensure that you wear your uniform whilst working and have your ID badge on your person
Ensure that you maintain the persons confidentiality and that your mobile phone is locked when not in use, that you ensure if you are talking on a telephone about a person there is no one near who can hear
Ensure the safety of the person by checking windows are closed, doors are not left open and key safe numbers are jumbled without anyone seeing
