This team does have a exception to work with those within the community, reviewing over their cases and assessing weather they have the appropriate funds in relation to their care package.

The successful candidate will have previous experience of working with relevant legislation. A Social Work (Degree/DipSW/CQSW) within a minimum of 1 year post ASYE experience is required. Experience within complex care, front door, community OP,PD and Hospital will be desired for this post.
