Rough Sleeper Outreach Worker - CoventrySalary: £15.34 per hourFull Time – Monday-Friday 09:00-17:00 (6am-2pm once a week)Role Purpose:
To provide support through outreach and street intervention working with support services and agencies to improve pathways for individuals who are street homeless to access suitable accommodation and services.To work as part of a team providing additional outreach capacity in order to work collaboratively with new or entrenched rough sleepers in Coventry. The outreach workers will be responsible for co-developing and monitoring support plans for rough sleepers on their caseload. To work with a defined agreed case load of up to 10 individuals at one time (up to 20 individuals over a year) providing intensive support that focusses on recovery.
Key Responsibilities:
As part of a team to provide outreach support and intervention to rough sleepers in a positive, persistent, proactive and assertive way to secure meaningful engagement to bring about sustained and improved behavioural change/outcomes for themContribute to holistic assessments of the needs of rough sleepers and contribute to the development of support plans accordinglyWork in partnership with existing agencies including police, housing providers, benefits agency, drug and alcohol services/workers, sex worker services as well as mental health and wellbeing servicesMake referrals to external agencies to meet the needs of those service users who need/ require additional input from other servicesContribute to the regular and consistent capture of data (hard and soft) to assess progress and performance for all individual support plansTo work with the Rough Sleeper team in establishing and implementing improved pathways for rough sleepers to exit street homelessnessParticipate in team, inter professional and inter agency meetings as required.To work flexibly across a seven-day week to meet the needs of the service and rough sleeper, including some early mornings, evenings and weekends.Any other duties and responsibilities within the range of the salary grade.

About the role

You will be working closely with children and their families, learning about their strengths and challenges and putting together an assessment of their needs.You will be a vital member of the team, skilled in providing a high level of professional childcare and social work practice using a wide range of therapeutic interventions.Your role as family support practitioner within the service will entail supporting young people to overcome and better manage difficulties they are facing by assessing needs, agreeing a service plan, carrying out agreed work and reviewing on a regular basis. You will work with families experiencing complex challenges and be creative in your approaches in engaging families who may be avoidant and resistant to your efforts.You will provide reports to referrers which critically analyse progress and outcomes achieved and which will inform decision making on children’s and their families circumstances going forward.You will be skilled in overcoming the challenges in maintaining and developing effective working relationships with external agencies to ensure the best outcomes for children, young people and their families.
The working hours are Monday to Friday, 9am – 5pm. It can include some early evening work to suit working parents.ESSENTIAL REQUIREMENTS INCLUDE:
Diploma in Health and Social Care Level 3 or above or equivalent/related health, counselling or social care qualification OR 5 years’ experience working in a multi-disciplinary team providing services to families and children aged 10-17 years.Extensive experience of working with young people and parents, preferably in age range 10-17.Experience of working with various issues particularly low level mental health issues (anxiety, self-harm, low mood) and low esteem etc.Good working knowledge of safeguarding and child protection procedures.Competence in the use of IT packages including MS Windows and Office along with email and internet and good record keeping.

About the role
We know that some domestic abuse victims and survivors in Camden need ongoing and additional support, once risk has been reduced and safety has been achieved. We know that the experience of VAWG intersects with severe disadvantage, increasing survivor’s vulnerability and posing additional barriers to recovery. Our model is predicated on assertive outreach, a trauma-informed approach, flexible 1-1 and group support which helps victims and survivors receive the support they need. Building on successful programmes, the VAWG Navigator team will be based in the community and will work at an individual survivor’s pace delivering high quality interventions along the continuum of need that enables greater safety in their accommodation or alleviates homelessness, improves engagement and recovery. The team will develop a strong constant relationship with survivors, working with a small case load to provide a person-centred offer, which promotes positive change; and coordinate the involvement of relevant agencies that can support the client
About you
Ability to build strong trusted strengths-based relationships with victims/survivors of VAWG and who have experienced multiple disadvantage
An understanding of the impact of VAWG on survivors and children
Experience of support planning and risk assessment
An understanding of cultural diversity and how to provide safe responses
Experience of maintaining excellent relationships and experience of building trusting partnerships across the multi-agency spectrum
Ability to advocate for survivors and their children
Ability to bring (to where?) the voice of survivors and lived experience of children to influence the system and need for change
You will be resilient and be able to work with people with multiple disadvantage
Work Environment:
The role will be based in locations around the borough, working with clients in their community setting. Working locations will be based on the needs of survivors e.g., hostels, outdoors.
