As a Community Nurse, you will play a crucial role in delivering high-quality care to patients within their homes, promoting independence and improving overall health outcomes.
Key Responsibilities:

Conducting comprehensive assessments of patients' health needs and developing personalised care plans.
Delivering a range of nursing interventions, including wound care, medication management, and symptom management.
Providing emotional support and practical advice to patients and their families.
Collaborating closely with multidisciplinary teams to ensure coordinated care delivery.
Maintaining accurate and up-to-date patient records and documentation.
Contributing to the evaluation and improvement of service delivery through feedback and reflection.

Requirements:

Registered Nurse (RN) qualification with active registration with the NMC
Previous experience working as a Community Nurse
Must be able to drive and have access to a vehicle.
Strong communication and interpersonal skills, with the ability to build rapport with patients from diverse backgrounds.
A compassionate and empathetic approach to patient care.
Ability to work autonomously and make sound clinical judgments.
Flexibility to work across bases if needed
