Adults Social Worker

Charles Hunter Associates are looking for a Adults Social Worker to join the Transfer of Care Team as a locum within East Anglia.
This post requires a Social Work Qualification of a minimum 2 year post experience.
About the team
This team operates within a Hospital Service, the team supports vulnerable adults that are within the Hospital setting.

A key contributor to the team function is to aid the individual, with the goal to leave the service and transfer back into the community.

Responsibilities can include, implementing care packages, practicing and carrying out assessments under the legislation of adult social services framework and working with multiple agencies for the benefit of the individual.
About you
A successful candidate will have understanding of working within adult social services within various team such as, community, hospital, complex care, safeguarding.

This post will require the candidate to have a social work qualification (Degree/DipSW/CQSW) with a minimum of 1 year post ASYE experience.
