PCN Clinical Pharmacist
A fantastic opportunity is now available for a Pharmacist to join a leading local GP practice team as a PCN Clinical Pharmacist.
Based a short way from Plymouth, you’ll join a well-established practice that serves both more built-up and rural areas. As part of a strong MDT (including doctors, nurses, paramedics, pharmacy and support staff), you’ll support excellent primary care such as by:
Conducting medication review clinics,Advising on and resolving queries regarding medicines and LTC management,Assisting with the resolution of prescribing-related issues,Supporting the treatment of acute illnesses, andParticipating in medicines optimisation and service improvement activities.As this will encompass travel between two surgeries, you will need a valid driving licence and access to a vehicle for this role.
This is a permanent PCN Clinical Pharmacist position, offering a high level of patient interaction in a positive and collaborative environment across four days.
Salary commensurate with experience, qualifications and training.
