Support Practitioner Apprenticeship

Provide support and guidance to children, young people, and their families in day-to-day activities. Assist in creating and implementing individualised care plans that promote well-being and development. Work alongside a multidisciplinary team to ensure the safety, safeguarding, and emotional support of young people. Complete and review incident reports, ensuring accurate documentation of events and actions taken. Participate in audits to ensure compliance with policies, procedures, and regulatory standards. Observe and report on the progress and needs of the children and young people, ensuring a person-centered approach. Engage in training, team meetings, and continuous professional development to enhance skills and knowledge.

Training:
Children, Young People and Families Practitioner Level 4 Functional skills if required Training schedule has yet to be agreed. Details will be made available at a later date.

Working Hours :
