This role will involve supporting people in the local community to live independently at home. The successful candidate will support customers with their day to day routine; Assisting with personal care, Administering medication, Preparing meals, Light domestic tasks, Carer relief and support out in the community. Work as an effective professional in a multidisciplinary Community Mental Health Team.
To prioritise and respond to urgent referrals for assessments as required and deliver high quality health and social care within the Care Programme Approach framework and to participate in duty rotas when required.
Hold care co-ordinator responsibility for a caseload of services users and their carers. Responsibility for assessing individual care needs and the planning, implementation and evaluation of care plans.
Work Monday to Friday, 9-5.
